Requests for medical records can be mailed in or dropped off to the Burbank Fire Headquarters at 311 E. Orange Grove Avenue, Burbank, CA 91502.
Some requests may take up to 10 days to process.
Requirements
- Complete the Request for Release of Protect Health Information
- Copy of identification
- $15.00 payment (payment options include cash, check, or credit card)
If you are requesting medical records for someone else, permission must be granted by the patient. If the patient is a minor, proof of guardianship is required (birth certificate). If the patient has a Power of Attorney, documentation must be provided. If the patient is deceased, a death certificate is required.
