How to Get a U.S. Passport

The Burbank Passport Facility can aid residents in getting a new U.S. passport. 
 

Services at our facility include:

  • Taking passport photos 
  • Requesting first-time passports or passport cards (includes Adults renewing a passport issued before they were 16)
  • Replacing lost, stolen or damaged passports/passport cards
  • Renewing passports - Minors only or if applicant was age (15) years old or younger
**All applicants must appear in person - Parents must appear with children** 


Adult renewals MUST be renewed by mail. Customers can mail their application and supporting documentation to the National Passport Processing Center.

First time applicants of all ages need Form DS-11.

Adult Passport renewals need Form DS-82.

To correct a data error, use Form DS-5504.
The passport card was designed for residents in northern and southern U.S. border communities that cross the border frequently by land.

The passport book is the only document approved for international air travel.

Passport cards are REAL ID compliant.

If you do have a Social Security Number, you must provide it on the passport application.

If you do not have a Social Security number, you will be provided with a declaration to fill out. 

If you fail to provide the information, you will encounter a delay in processing and/or denial of your passport application. 
If you have a life-or-death emergency and are traveling internationally in the next 72 hours (3 business days), please visit the U.S. Department of State's Life-or-Death Emergency page for more information.
